Application and Fee Schedule
You can download and print the 2013-14 enrollment application and fee schedule (additional copies are available in The Day School office). Application forms for the 2012-13 school year are available in The Day School office only.
Required Forms
Once your child is enrolled, the following forms must be completed before their first day of school. These forms will also be available during Parent Orientation in September.

The Commonwealth of Virginia School Entrance Health Form must be dated and signed by a doctor, in accordance to the Commonwealth of Virginia health department. The health form must be current and updated annually.

Proof of Birth
All new students must provide Proof of Birth (passport, original birth certificate) by the first day of school. We cannot accept photocopies.
Parent Handbook
The Parent Handbook is available for all parents of newly enrolled students. Copies may be obtained in the Day School office or by clicking on the link below. Please refer to the handbook for the policies and procedures set by the Committee of The Falls Church Day School.
